About the Role
Join the Assertive Community Outreach Service (ACOS), an intensive Community Mental Health Team in the High and Complex Needs Services.
About the Team/Service/ Location
Our multi-disciplinary team work with people who need support to engage with mental health services and present with high risks associated with their mental health. We aim to provide flexible, collaborative and responsive treatment and support to our Service Users/Tangata Whai ora. We are a mobile team based in Point Chevalier, Auckland that covers all of the Auckland District Health Board area.
We provide comprehensive mental health support with a focus on whanau/family involvement. You will apply your mental health knowledge and skills to undertake thorough assessments, including risk management and provide appropriate and collaborative treatment and support using a case management approach. As an ideal candidate for this role, you will be keen on taking the initiative, enjoy teamwork and have a good knowledge of community resources.
This is a permanent, full-time role working 10 days per fortnight on a Monday to Sunday roster. You will be working one or two weekend days per month, as agreed
