NASC Triage Clinician - Needs Assessment & Service Coordination, Adult Community Services
Permanent, Full Time (1.0 FTE) Monday to Sunday rostered shifts

- Are you keen to utilise your experience and expertise in NASC triage?
- Do you have exceptional communication skills?
As a NASC Triage Clinician you will be responsible for timely and accurate NASC triage and coordination of referrals to identified services, facilitating the appropriate pathway and workforce as part of the Adult Community Services single point of triage we are working towards.
We are looking for a registered health professional who is flexible, resilient and can manage a busy workload. As an ideal candidate, you will be InterRAI trained and have 3-5 years experience as a District Health Board (DHB) NASC Service Coordinator.
Your clinical decision making and communication skills along with proficient organisation and computer skills are imperative to be successful in this role.
Adult Community Services includes Needs Assessment and Service Coordination, Allied Health, Community Rehabilitation, Intermediate Care, Mobility Solutions, Specialist Nursing and District Nursing. There are approximately 230 clinical staff in community services that are focused on providing quality health outcomes.
